Appointments

Scheduling an Appointment

All patients must have a physical therapy referral (SF513 Consultation or a referral placed in the electronic medical record) requesting our service. Consults or referrals greater than 30 days old will not be honored and will require renewal by the referring provider unless arranged through the Chief of Physical Therapy.

Treatment appointment frequency will be determined by your physical therapist. All treatment appointments are scheduled on a weekly basis. A follow up with your physical therapist is required every 30 days while you are receiving treatments in our facility.

For Your Appointment 

If you are coming for an initial evaluation or a follow up with a PT, please arrive 5-10 minutes early to fill out paperwork. If you are more than 10 minutes late for a scheduled appointment, your appointment will be rescheduled.

For treatment appointments, it is not necessary to arrive early, however if you are more than 10 minutes late for a scheduled appointment, your appointment will be rescheduled.

For all appointments (initial evaluation, follow up and treatment), please wear appropriate clothing (physical fitness uniform or civilian workout clothes).

Children are not allowed in the treatment area, and must be accompanied by an adult in the waiting area; if this is not possible the appointment may be rescheduled.
 

Rader Physical Therapy is now offering patients the ability to download and print their initial intake questionnaires prior to the first appointment to save time in the waiting room.

Please select from the following links for the body region that best corresponds to what you will be evaluated for by the physical therapist. Please print and bring the completed form with you to your first appointment. If you are unable to print or limited on time, you will still have the opportunity to complete this paperwork in the clinic the clinic. Note: lower extremity includes hip, thigh, knee, shin, ankle and foot.

U.S. Army Physical Therapist Scope of Practice

  • Evaluate, diagnose and treat patients with neuromusculoskeletal injuries across the full spectrum of evidenced-based practice
  • Consult with other providers on musculoskeletal or neuromusculoskeletal patient management and disposition
  • Conduct clinically relevant research
  • Optimize Soldier/unit readiness by enhancing functional/human performance
