Physical Exams

Eligibility

  • Active Duty or Active Guard and Reserve (AGR) personnel enrolled to Andrew Rader US Army Health Clinic as their primary care facility.
  • Active Duty or Active Guard Reserve AGR Army personnel who are assigned to an Air Force base or Navy base within the National Capital Area, or a health care facility that does not have Medical Protection System (MEDPROS)  Periodic Health Assessment entry capability.
  • Active Duty Marine, Navy, and U.S. Public Health Service assigned to Rader Health Clinic. Navy personnel and Marines needing any type of special physical (schools, demobilization, etc.) should provide their Services' requirements for those exams.
  • DOD civilians are eligible to complete Pre-Deployment physicals at Rader. They must be enrolled in TRICARE before scheduling an appointment. ensuring they are enrolled in the system for Rader Health Clinic. All DOD civilians are required to provide all forms necessary for the physical and a list of requirements.
  • Foreign Nationals enrolled at Rader are eligible. They should provide any host nation forms required for the physical examination as well as a list of requirements.

All other situations will be considered on a case-by-case basis per the officer in charge of physical exams. All patients are seen by appointment only which are booked by calling 703- 696-3477. You cannot book physical exam appointments using the patient portal.

Services Offered

Periodic Health Assessment

Army, USMC and Navy

  • Call to schedule your Part 1 appointment.
  • If you are a Male 35 or older, Female 40 or older you must Fast 12 hours prior to your appointment.
  • You must complete the on-line portion of your PHA by logging on to your Enterprise account and clicking on the drop down box My Medical Readiness. It is mandatory to complete this prior to your Part 2 appointment.
  • Part 2 is the review of your Part 1 Exam and on-line Periodic Health Assessment. This exam can only be reviewed by a PA/NP/MD.
  • The completed Physical can typically be picked up approximately one week after the Part 2 exam is complete.

Post-Deployment Health Assessment / Post-Deployment Health Risk Assessment

Physicals

Welcome Specialty Clinic Physical Examination. The information contained in this page is only applicable at Rader and not at any other military hospitals or clinics. Our Physical Exams offer the following physicals:

  • Commissioning / Appointment:DD2807-1 / DD2808
  • Airborne: DD2807-1 / DD2808
  • Ranger: DD2807-1 / DD2808
  • Special Forces: DD2807-1 / DD2808
  • Separation (ETS / Referred): DD2807-1 / DD2808 / DD2697 / ETS Letterhead
  • Chapters (Must have escort): DD2807-1 / DD2808 / DD2697
  • Retirement: DD2807-1 / DD2808 / DD2697
  • Others on a case-by-case basis

Part 1 – Call to schedule your part 1 appointment. Be sure to Fast for 12 hours prior to your appointment. The purpose of the Part 1 is to complete all the required forms, Lab test, X-Rays, EKG if required, Hearing and Vision Screening.

Part 2 – Will be scheduled ONLY after all Lab results are completed.

Audiology

Audiology Clinic (hearing test) is for Active Duty only, within the Specialty Clinic. Audiology is currently open by appointment only. 

Veterans Affairs Physical

If you are planning to file a Veterans Affairs (VA) claim for an injury or illness that is a result of military service, you can receive the separation and VA compensation physical examination at the same time.

NOTE: Your VA compensation physical examination can only be done through the VA.

To combine your separation and VA physical and file a VA claim, you must begin the process well before you will leave the service. You can schedule this exam up to six (6) months (180 days) prior to the scheduled retirement date. Examinations will be required no later than 90 days prior to separation/retirement date. Contact the VA Regional Office nearest you or by calling the Veterans Crisis Line 1-800-273-8255 and Press 1.

If you do not meet the time deadline:

  • Complete the VA disability compensation claim before final out-processing.
  • Attend the VA disability claims briefing.
  • Turn in the VA compensation claim for with your service medical records during final out-processing.
  • The claim and your medical records will be forwarded to the Veterans Administration Regional Office (VARO) having jurisdiction over the address provided on the claim form. The VARO will then schedule any additional medical examination required to complete processing of the VA claim.
  • If you do not complete a VA disability claim before final out-processing you can contact the VARO by calling 1-800-273-8255 after separating from service. VARO will provide you with the disability claim from and other necessary information. Although a claim may be filed at any time after separation, it is recommended that the initial application be completed within one year of release from service.

Service members must have a tetanus shot that contains "pertussis" regardless of when your last tetanus shot was.

You can look for the following on your immunizations record to ensure that you are up to date on your immunizations.

  • Two Hep A shots spaced 6 months apart
  • Three Hep B shots with the first and second being spaced by 1 month and the second and third by 5 months.
  • Annual Flu vaccine
  • Tetanus Vaccine that contains pertussis. Also known as TDAP.
